			<content:encoded><![CDATA[<p>Plastic surgery is common place in western nations today, with most registered plastic surgeons offering a sound and professional service to their patients. Plastic surgery does however involve serious surgical procedures that should not be undertaken lightly. When undergoing any plastic surgery at all it is crucial that the patient is aware of all of the possible risks and complications. While the rate of complications among certified professional surgeons is low, it is still important for potential patients to be informed about any possible dangers before they begin.<span id="more-249"></span></p>

<p>The field of plastic surgery (Plastische Chirurgie) involves both reconstructive and cosmetic surgical procedures. However as reconstructive surgery is generally used to directly improve a persons physical functions, the risks involved are normally recognised as being more acceptable to the patient. There are of course many risks involved with reconstructive surgery as well, however for this reason I will not focus on them in this article. While there is no doubt that cosmetic plastic surgery can have a positive influence on a patient, the associated risk factor is more complicated due to the elective nature of the surgery.</p>
<p>The vast majority of cosmetic plastic surgery by certified professional is safe for the patients and can lead to positive physical and mental health outcomes for people. The risks involved are generally specific to the particular type of surgery being carried out, for example <a href="http://www.beauty-pro.de/facelift_berlin.aspx" target="_blank">facelifts</a> have very different risk factors than liposuction or breast augmentation. Before I mention specific surgery however, there are some general risks that are involved with any surgical procedure – cosmetic of not. Some patients have complications due to anesthesia and sedation procedures. Some other possible complications include an abnormal heart rhythm, airway obstruction, blood clots and stroke. Although such complications are rare, any patients with heart trouble, lung disease or obesity are at a higher risk.</p>
<p>Other possible but unlikely complications that can arise during any surgical procedure include aspiration, blood loss, blood clots, blood pressure problems and possible infection. The amount of risk involved in surgery is of course dependent on the health of the patient and the type of surgery being undertaken. As well as these general factors, cosmetic surgical procedures do carry some risks of their own. Procedures involving the skin of a patient can lead to skin death or necrosis, a situation that usually arises after an infection and is much more likely among smokers. Slow healing may also be an issue for the occasional patient after some procedures.</p>
<p>Moderate to severe physical asymmetry can result from an unsuccessful procedure, however a lot of these problems can be addressed during a second procedure. Numbness and/or tingling in certain areas is another possible risk of cosmetic plastic surgery, this if often only temporary but can be permanent at times. One other risk I will mention is called ’seroma’ and results from fluid collecting under the skin after breast augmentation, liposuction or a tummy tuck. Post surgical depression is another risk that some patients suffer from, however a lot of patients are also very happy after their procedures and the changes that they have made.</p>
<p>Any responsible professional plastic surgeon should mention all of these possible risks while they are consulting about a possible procedure. There are also other risks that are specific to the type of surgery taking place. It is important to remember however that plastic surgery that is performed in the correctly accredited and recognised facilities, by professional and certified surgeons does have a very good safety record. The rate of serious complications is very low and is often seen as being outweighed by the potential benefits of cosmetic surgery. However whether you are undergoing surgery for reasons of beauty or health, you can never be too careful. A patient needs to be informed and also to keep their surgeon and surgery team informed about their medical history as well as any psychological considerations.</p>

<p>Out medical centre has a strict uniform policy so I usually have to wear a uniform as part of my job, this is black trousers, a white shirt and a burgundy scarf. We have to have an identification badge, which is also worn around the neck and the photo usually covered by the Scarf. Plus we have to wear our hair up and black shoes.</p>

<p>On Friday however, we are having a ‘jeans day’ at work, where everyone who wants to can put 1 pound into a charity pot and can then wear jeans all day on a Friday. I was the first to say yes to this, as I hate walking around in uniform, it is far too formal and I think often puts patients off talking to you.</p>
<p>So I put my pound in the pot, and went home to try and find suitable jeans to wear for the day. I had all sorts of designer labeled women’s jeans, some of them had skinny leg, some boot leg, others were three quarter length jeans. I had different colours also, black, white, blue, dark blue, stone wash. You name it and I had it in my wardrobe. I didn’t want to look too over dressed so I decided that the black ones were not even in the equation when choosing.</p>
<p>I firstly decided which top I was going to wear on non-uniform day, then I could match the top up with the jeans. I chose a top which was casual but still looked quite smart. It was checked and had all different colours in it, such as orange, purple and white. I tried it on with all of my jeans, but I felt that it matched my Tommy Hilfiger stone wash jeans best. They were boot cut and looked really good against the material of the shirt.</p>
<p>I put my cowboy boots on with them, I was going to wear trainers but I felt the whole look would then look too casual, like a Used Cars for Sale.</p>
<p>When I got to work the next day all my colleagues had decided to take part in ‘jeans day’, I had a lot of positive comments about my Tommy Hilfiger women’s jeans. Some of the other girls there also were wearing Tommy Hilfiger jeans.</p>
<p>From now on I will be taking part in jeans day every week at work, I found the whole atmosphere in the medical centre more relaxed. The jeans in general were much more comfortable to wear than the uniform usually was. Plus the money they raise every week through people handing in a pound will all add up, and will help the children’s charity immensely with their work. My only hope is that our managers decide that we can wear our jeans all through the week, and then occasionally we can give in a pound to wear our uniforms instead, but somehow I don’t think they would raise as much money for charity.</p>

It can be a very difficult time for all involved and the last thing you want to have to deal with are accommodation or transportation problems, so it is a good idea to deal with these things first and get them out of the way. Staying in a centrally located holiday apartment is often the best and most relaxing alternative when visiting a sick relative in Berlin. If you are not stressed out about the technicalities of your visit then you are more likely to have energy available to help your sick relative to get better, or at the very least give them some peace of mind. As far as big metropolitan cities go, Berlin is quite an easy place to get around, and as long as you have a well located Berlin apartment you should not have any difficulty getting to your location, whether it is at a hospital or elsewhere.</p>

However the act of just being there for someone who is sick can be incredibly healing and helpful to them, and seeing your familiar face can really help to get them through their day. It is also a good idea to talk to your relative about yourself for a while; many people think that if they talk about themselves they are being selfish, but it really is quite the opposite.</p>
<p>If you can talk about yourself and what is going on in your life, it can make your relative forget about their troubles for a while and this will probably be a great help to them. You just have to be relaxed and be yourself, although sometimes this is easier said than done. By just being there for your sick relative you are doing so much to help, this along with the fact that you have traveled especially to be with them will make them feel loved and special, and that really is the best thing you can do for anyone.</p>

For elderly people who need to visit day care medi-cars are on hand to pick them up from their front door and drop them back off when they are ready to return home.<br />

<p>For orthopaedic surgery, such as the replacement of hips and knees, and certain spinal operations, a stay in hospital is required for after the procedure. These are done through the NHS or you can pay to have these privately. There is now a scheme called choice which is run in Sheffield, which means you are able to choose where you have a procedure done. So you can choose to be seen by a consultant in a private hospital, plus you can have your operation done within this hospital. The service is exactly the same as you would expect in the NHS, except you get to stay in the luxury of a private facility.</p>
<p>If you are having a heart bypass, then this kind of surgery usually takes 3 to 5 hours. An incision is made using a surgical knife down the centre of the chest, this allows the surgeon to cut through the chest bone, in order that the rib cage is opened to enable access to the heart.</p>
<p>With any of the above surgical procedures the recovery process is often worse than the operation itself. A team of dedicated nurses are on hand day and night to assist with this during your stay in hospital. Physiotherapists help to get the patient mobile again, helping the patient motivate themselves shortly after the operations, plus to make sure that the patient can walk up and down stairs before they are deemed fit to return home.</p>
<p>Because of the MRSA which is now present in our hospitals throughout the United Kingdom there is a big drive on cleanliness within the hospitals. Hospital hygene is now paramount within the healthcare service to prevent this disease spreading, MRSA can be deadly particularly in the elderly or the vulnerable patients. Antibacterial spray is available before entering any ward in a hospital now. It is also available for use at the end of each patients beds, and is recommended to be used by staff before dealing with a patient, and by visitors to the ward.</p>
<p>In maternity hospitals hand hygene is also on the adgenda, as babies are susceptible to any sort of infection, these hand sprays are on each of the mothers beds for use by staff and visitors.</p>
<p>Before knives and surgical instruments are used on any patient, the issue of sterialisation has already been dealt with, all surgical implements are steralised, and some are even disposable to prevent any sort of cross infection.</p>
<p>Whatever procedure the patient has in hospital, they can usually be guaranteed the best of care by all of the staff involved in the patients operation, and after in the recovery processes. Plus the food in hospitals has improved over the last few years, the patient now gets a choice of what they wish to eat, the food even coming with a salt and pepper mill.</p>

<span id="more-238"></span>Although this is a good thing now, in the past when a person got old there was little available in the way of care and technology to help that person stay within the comfort of their own home. 			<content:encoded><![CDATA[<p>In the United Kingdom if a patient has a health problem the usual place the patient would visit would be your GP, if the problem needed specialist treatment then the patient would be referred to see a doctor or consultant on the NHS.  Unfortunately there are often long waiting lists to see certain specialists.  The alternative for a patient wanting to be fast tracked would be to go to the same specialist within the private sector.  This way the patient could get a diagnosis and if the consultant felt that the patient needed further treatment or surgery the patient would be given the option to have this done either privately or on the NHS.  The consultant who gives you the diagnosis is usually the surgeon.</p>

<p>If a patient were needing joint treatment (Arthrose Behandlung) then the patient would need to see a Consultant who specialises in orthopaedics (Orthopäde).  The consultant will make a decision at some point during the consultation as to whether the patient needs an x-ray, if the patient has a knee or hip complaint then they would be sent for a plain x-ray (standard), this would then be read by a consultant Radiologist who would determine what was wrong with the patient from the x-ray.  If however the patient was to have a back complaint then the consultant might find it necessary to perform an MRI scan.  An MRI is magnetic scanning which gives no radiation to the patient, a patient is required to be very still throughout the scan in order that clear sequences can be obtained.  Unfortunately some patients cannot tolerate the scan due to claustrophobia.</p>
<p>Once an x-ray has been obtained by the consultant, then a decision will be made as to whether or not the patient requires surgery.  If the patient has come for a private consultation but cannot afford the operation to be done privately then the patient will be referred back on to the NHS.  They will get the operation done faster than if they had gone through the whole procedure on the NHS as they have already acquired a diagnosis.</p>
<p>If the patient is to have surgery then they would be asked to attend a pre-operative assessment about a week before their operation date.  During this assessment the nurse will take bloods, urine, blood pressure and if needs by a chest x-ray to determine whether the patient is deemed fit for the operation.  The patient will be given instructions with regards to fasting before the procedure and the surgery usually takes place on the day of admission (with some surgery the patient is taken in the night previously for rest and preparation)</p>
<p>Once the patient has had the surgery then they will be x-rayed again (a post operative x-ray) to ensure that everything is in place and that no complication has occurred at the site of surgery.  They will then be given medication and pain relief by the nurses and the physiotherapists will help the patient to get back on their feet, before sending them home to recover.  They will be asked to attend a follow up appointment usually 6 weeks after surgery, so that the consultant can sign the patient off.  The patient is then asked to attend for a 1 year post operative x-ray to assure that everything is still in situ and as it should be.</p>

<p>From here the clothes are distributed around the UK, to non profitable organisations (such as charity shops). Where they are priced up, and hopefully sold on, in order to help the charity in which the shop supports. Some of the clothing that is received often has a brand name (<a title="Djeco" href="http://www.ichbinklein.de/index.php?manufacturers_id=62" target="_blank">Djeco</a>).</p>
<p>If the charity shop doesn’t manage to sell the products in the shop, then they are then shipped on for use in third world countries.</p>
<p>When the clothing lands at its destination, it is usually taken to an enormous distribution centre, where every used item is sorted. Some of the clothing can be sold on in the third world, the other clothing is distributed to people who need it, and last of all if it felt that the clothing is of no use, then it will be recycled.</p>
<p>However, although this all sounds good over 30 countries have stopped the importation of this clothing. Those who haven’t banned it, have put other restrictions in the way of clothing reaching the needy, these include expensive licenses and high tariffs, which unfortunately make it to expensive for the companies with the clothes to ship them.</p>
<p>The reasons behind the ban vary, some countries feel that there dignity is taken away from them if they accept second hand clothing. However, these clothes are much needed for people in third world hospitals, and the people who cannot afford new clothes. For example if a Bolivian man was to buy a new shirt in his country, it would cost him around 70 Bolivan pesos, if he was to buy a second hand shirt it would cost him 5 pesos.</p>
<p>A lack of health education in deprived countries has also lead to these clothes being banned. Five years ago Cameroon wanted to ban the importation of second hand clothing fearing that it would help the spread of AIDS/HIV, and skin infections. If a little money was put into educating, this situation may not exist.</p>
<p>However, the clothing that does get through to the babies in third world hospitals, is well used and appreciated. It often means that babies who have come from a deprived back ground are given baby clothing (<a title="Baby Kleidung" href="http://www.ichbinklein.de/" target="_blank">Baby Kleidung</a>) which is equivalent to pajama’s, for while they are in hospital. On the patients discharge they are given clothing, which is used for outside wear.</p>
<p>So while the government worries about the spread of infections, and whether they loose face with the rest of the world by accepting second hand clothing. Providing baby clothes for third world hospitals certainly is of benefit for the babies who are born into hardship and have little. It also goes a long way to help the parents of these babies, who struggle on a day to day basis to provide essentials, such as food and water.</p>

<p>Daisy was the most beautiful lady he had ever laid eyes upon. She had pale skin and red lips; her hair was like the fire of a volcano. Tommy was hooked from the start. He loved the way she talked; he loved the way her eyes would twinkle when she smiled. There was just one thing wrong with Daisy. She was taken, or more appropriately held down, by Andy Papowitz – the biggest sleaze this side of town.</p>
<p>Andy Papowitz was a big time moneygrubber from Detroit who had his hand in everybody’s pocket. His most recent endeavor was laundering funds through private equity. He was making a large profit practically stealing millions of dollars right under people’s noses.</p>
<p>Nevertheless, Tommy just could not stay away from Daisy. He was in love. He had to be with her, to touch her face to kiss her lips. There was no way he was letting a squalid fool like Andy Papowitz stand between him and his one true love.</p>
<p>Tommy began going to the hospital all the time to see Daisy. She found herself intrigued by his charm and soon felt the same love as he. The bond they shared was like nothing either of them had ever felt before with anyone else. They decided that would run away together, and be together forever.</p>
<p>Daisy did not answer the phone when Tommy called from the driveway so he went up to see what was taking so long. The door to Daisy’s apartment was open so he went in and found Daisy lying on the floor in a pool of blood. She has been stabbed to death. Tommy knew right away that Andy Papowitz was responsible. The police soon knew as well when they found Andy Papowitz’s cigarette with his DNA all over it still burning beside the door.</p>
<p>When they went to arrest Andy Papowitz, he was nowhere to be found. All of his belongings remained in his house. They looked for him for a couple of years and then just let it go. Little did they know Andy had kept a secret wall in his basement where he hid all of his money from his private equity scam. Unless someone knew it was there, they would not know to look for it. He had bragged about it to Tommy one night after too much Tequila.</p>
<p>Three days after Andy Papowitz disappeared, an oversized suitcase containing $800,000 was left outside the local hospital with a note that just said In Loving Memory of Miss Daisy Fuqua.
